How is the score calculated?
This is Nick’s Software’s own ten-question discussion aid. It is not presented as a recognised maturity standard, external audit or validated industry benchmark.
- 1. Points per answer
- Each option = 1, 2, 3 or 4 points
- The points are shown beside every option. All questions have the same weight; there are no hidden multipliers.
- 2. Total points
- Sum of the ten selected option values
- Minimum 10 points; maximum 40. Each extra point changes the unrounded score by 2.5 on the 100-point scale.
- 3. Displayed score
- round((total points ÷ 40) × 100)
- Rounding is to the nearest whole number, with .5 rounded up. The score is not a percentage of digitised work or an industry percentile.

What happens if every answer scores 2?
This is an arithmetic example—not a suggested factory profile or typical result. The assessment starts without any selected answers.

Two operations can receive the same total while having different response profiles. Read the answer breakdown before choosing an improvement.
Assumptions & limitations
Treat the result as a starting point for checking actual workflows, records and outcomes with the team—not evidence that a factory is ready for a specific investment.
- Self-report, not verification. The tool scores the descriptions you select. It does not inspect machines, systems or records, or test whether the stated capability works reliably.
- Technology-heavy choices can score higher. This reflects the questionnaire’s design, not proof that more automation or AI is appropriate. A controlled manual process may fit a particular operation; do not buy technology just to improve the score.
- Equal weights can hide important gaps. A higher answer in one area can offset a lower answer elsewhere. The total does not identify a constraint, a required prerequisite or the highest-value project.
- No not-applicable option. All ten questions must be answered. If an area does not fit your operation or you cannot assess it reliably, record that caveat and do not treat the overall score as representative.
- Critical checks are separate. This questionnaire does not assess cybersecurity, functional safety, regulatory compliance, recovery, investment costs or business-case returns.
- Comparison needs a consistent scope. Compare reassessments only when the factory or process, answer definitions and evidence basis are compatible. The score is not a ranking against other businesses.

Know what the score measures—and what it does not.
How is the digital transformation score calculated?
Choose one answer for each of ten questions. Answers are worth 1, 2, 3 or 4 points, and every question has equal weight. The tool adds the points, divides by the maximum of 40, multiplies by 100 and rounds to the nearest whole number. It does not calculate an industry percentile.
Why do the lowest answers give 25 rather than zero?
Each question starts at 1 point, so ten lowest-point answers total 10. Dividing 10 by 40 gives a score of 25 out of 100. Complete assessments can score from 25 to 100. This preserves the existing model; the score is not a percentage of work that has been digitised.
What are the stage thresholds?
The rounded score determines the label: below 35 is Foundation Stage, 35–54 is Developing Stage, 55–79 is Advancing Stage and 80–100 is Leading Stage. These are this tool’s own bands, not independently validated certification levels or comparisons with other factories.
Can I change an answer or leave a question unanswered?
You can go Back and change a selection, or edit your answers after seeing the result. All ten questions need a valid selection before a score is shown. Changing a completed assessment marks the old result as not current until you calculate again. Reset clears the selections and result.
Does a high score mean my factory is ready for AI or automation?
No. The questionnaire is a self-reported discussion aid. It does not test cybersecurity, safety, data quality, system reliability, regulatory compliance, return on investment or whether a particular technology fits your operation. Review those separately before committing to a project.
Do I need to give my email address, and what happens if I send a request?
No email is required to view your score and response breakdown. The tool does not save your selections between page loads. The optional review request sends your email, score, stage and selected answers to Nick’s Software through Web3Forms for follow-up. It does not generate or promise an automatic detailed report; the production site’s normal analytics also applies.
